PLEASE NOTE: This event will take place at Bojangles Coliseum in Charlotte, NC. We will send an email a few days before the event for those wanting to carpool to the Coliseum.

EMERGENCY MEDICAL AUTHORIZATION: In the event of an illness or injury requiring medical attention, I authorize South Point Baptist Church, its staff members, and approved adult volunteers to seek appropriate medical treatment for my child. I understand that reasonable efforts will be made to contact me as soon as possible in the event of an emergency.
LIABILITY RELEASE: I, the parent / legal guardian / legal participant, through the submission of this form, do hereby release, acquit, hold harmless, and forever discharge South Point Baptist Church (henceforth referred to as SPBC), its agents, servants, and employees, and all persons, natural or corporate, in privities with them or any of them, from any and all claims or causes of action of any kind whatsoever, including but not limited to actions, suits, and/or claims for any bodily injuries, death, or property damage which may be sustained by the participant listed above while participating in any Children/Student/Church Ministry activity, or activities, (including travel to and from such activities) resulting from the negligence or lack of care due or claimed to be due to the conduct of any agent, servant, or employee of the SPBC, for any and all activities.
TRANSPORTATION RELEASE: I understand that transportation for this event will be provided by approved adult drivers who are parents or staff members of South Point Baptist Church. These individuals are not commercial transportation providers. I understand that drivers are expected to maintain valid driver’s licenses and personal automobile insurance coverage. I grant permission for my child to ride in vehicles driven by approved adult drivers for the purpose of attending this event.
